It begins with listening.

When onboarding a new client, our care managers work with them and their families to understand the full picture and guide them through all available programs and services.

Then comes collaboration.

We work closely with each patient’s referring care managers and collaborate with their other caregivers to keep everyone working as one unified care team.

Next is a 360° assessment.

We perform a thorough, one-on-one assessment that accounts for the needs of both the individual patient and their entire family unit.

Finally, we shape a personal plan.

After developing a customized care plan, we individually assign dedicated nurses and therapists to integrate into the patient’s home setting, family dynamic, and care team.

Our care is delivered by highly trained RNs (Registered Nurses) and LPNs (Licensed Practical Nurses)

24/7 Coverage

We provide 24/7 nursing coverage to support patients and families at all times.

Form 1095-C for an eligible employee may be requested from the company by emailing HR@gnyservices.com, call 212-687-7464 or requesting a copy in person at our main address of 6424 18th Avenue Brooklyn NY 11204. It shall be furnished to the employee within 30 days of the request.
