When onboarding a new client, our care managers work with them and their families to understand the full picture and guide them through all available programs and services.
We work closely with each patient’s referring care managers and collaborate with their other caregivers to keep everyone working as one unified care team.
We perform a thorough, one-on-one assessment that accounts for the needs of both the individual patient and their entire family unit.
After developing a customized care plan,
we individually assign dedicated nurses and therapists to integrate into the patient’s home setting, family dynamic, and care team.
